Pre-Registration Requirements

The following is a list of forms that are due in The Registrar's Office by July 15, 2009.  These forms are provided below for your convenience in the event that you may need duplicates of the forms. Please return completed forms to the Fletcher Registrar's Office as soon as you are able.

Health Service Office Requirements:

In addition to the forms required by the Registrar's Office at The Fletcher School, The Health Service Office at Tufts requires that you provide them with documentation prior to your enrollment.  Please see below for the links to the required Health Examination Report as well as general information regarding requirements.

  • Third Party Billing:

    Tufts e-Bill is not for use in billing 3rd party sponsors, scholarship organizations or grant administrators. If a third party sponsors you (meaning that tuition and/or fees should be billed directly to the sponsor rather than yourself), please make sure that the Bursar's office has the proper documentation. This documentation would include what is to be billed to the sponsor, the duration of the sponsorship and the sponsor’s billing address.

    The following people handle third party billing:

    Please contact the designated person to determine if they have the appropriate documentation on file so that they can bill your third party.  All can be contacted through the Student Services Desk at Dowling Hall (617-627-2000).  Since you are ultimately responsible for your bills and making sure that they are paid on time, you will always receive a copy of your e-Bill.  It is your responsibility to check it for accuracy and ensure that it is paid in full.
